Why information is collected

Appointment requests collect your name, phone number, email address and preferred date/time so the practice can contact you about visit arrangements. General enquiries collect name, email, optional phone, enquiry category and message to respond to your question.

Please do not submit clinical information

These forms are for administrative matters only. Please do not provide identity numbers, diagnoses, symptoms, prescriptions, test results, medical records or other sensitive health information. Contact the practice to establish an appropriate secure channel for clinical correspondence.

Processing and sharing

Submitted information is stored in a restricted Cloudflare D1 database for authorised practice access and may be transmitted to a transactional email service to notify the practice and patients. Cloudflare Turnstile checks submissions for abuse. These service providers may process information outside South Africa; the practice should verify applicable privacy and cross-border arrangements before enabling the forms.

Retention and requests

Practice administration may remove appointments and enquiries when no longer required. Contact the practice to ask about access, correction or deletion, subject to any applicable legal obligations. Automatic deletion is not enabled in this release.

Cookies and local storage

This site uses a privacy-notice preference stored locally in your browser and an essential restricted administrator session cookie if an authorised staff member signs in. It does not contain advertising analytics in this build. Cloudflare security services may use technical cookies for abuse prevention.

Emergency requests

Online requests are not monitored continuously. Do not use website forms in an emergency.

Information Regulator

You may contact South Africa's Information Regulator regarding unresolved privacy concerns. This notice should be reviewed and approved by the practice's Information Officer prior to accepting live submissions.
